He's never been allowed a visitor before and he's hardly been the recipient of any sort of gift, so there's very strong and plain surprise present. Kaoru's gaze darts briefly toward the door that the servant is most certainly still waiting on the other side of, like he's expecting something; for the bag to be snatched away, perhaps, or for instructions on how to accept it since he's floundering on his own.

But even without direct instructions on how to gracefully receive a gift - the Nagumos apparently weren't expecting it, either - he tries his best to make do with what he does know about manners.
